WHAT TO KNOW ABOUT HEALTHCARE IN SPAIN

EU citizens with a valid EHIC card can access public healthcare. Non-EU residents need either private insurance or to register with the public system via their padron and residency status.

Private health insurance in Spain is generally very affordable compared to the UK or USA — from around €50–100 per month for adults.

A médico de cabecera (GP) is your first point of contact in the public system. For private care, you can go directly to a specialist.

COMMON QUESTIONS

Healthcare questions answered

The things people ask us most often about healthcare in Seville as a foreign resident.

Can I use the Spanish public health system as an expat?

It depends on your situation. EU citizens with a valid EHIC card can access public healthcare during their stay. If you’re a registered resident, you can access the public system once you’re registered on the padron and have your TIE. Non-EU nationals on certain visas (including the digital nomad visa) may need private health insurance as a condition of their visa. If you’re in any doubt about your entitlement, a gestoría or residency specialist can clarify your specific situation.
Private health insurance in Spain is genuinely affordable compared to most other countries. A healthy adult in their 30s can expect to pay around €50–80 per month with providers like Sanitas, Adeslas, or Asisa. Prices rise with age and pre-existing conditions. The main benefit beyond cost is speed. Private appointments are typically available within days rather than weeks, and you can go directly to a specialist without a GP referral. Many expats maintain both public entitlement and a private policy.
A médico de cabecera is a GP and your first point of contact in the public health system. They handle routine consultations and refer you to specialists when needed. In private healthcare you can usually go directly to a specialist without a GP referral, which is faster for specific issues. Specialists (especialistas) cover areas like cardiology, dermatology, gynaecology, and orthopaedics. If you’re using private insurance, your policy documents will list which specialists are covered and how to book directly.
Sanitas and Adeslas are the two most commonly used by expats in Seville and both have strong networks of clinics and specialists in the city. Asisa is another solid option. Cigna Global is popular with higher-earning expats who want international coverage that works across countries, not just Spain. If you’re applying for a visa that requires health insurance, check that your chosen provider’s policy meets the minimum coverage requirements as not all standard policies do.
For most non-EU visa applications, including the digital nomad visa and non-lucrative visa, private health insurance is a required document. It needs to cover the full duration of your stay in Spain with no co-payments, and must meet a minimum coverage amount set by the Spanish government. EU citizens applying for residency under freedom of movement rules don’t typically need to show private insurance, but do need to demonstrate they won’t be a burden on the Spanish public health system. A residency specialist can advise on exactly what your specific application requires.
